Following admission, each resident is assessed by our Interdisciplinary Team comprised of a medical doctor, nurses, dietician, social worker, therapists, and therapeutic recreation staff.
After the initial Plan of Care is determined, a Plan of Care conference is held quarterly as well as in the event of any significant change. In addition, an annual assessment is provided to monitor the progress of your loved one.
